By joining Kilauea Phys Ed and choosing the regular 12 month recurring membership, I thereby agree to the terms and conditions outlined here:
12-Month Autopay Memberships:
After the initial monthly fee and enrollment fee are paid. I agree to a monthly Autopay charge of $99 monthly via a credit or debit card beginning 30 days after the initial payment. I agree that I am a duly responsible authorized account holder on the credit/debit card issued by the presented card’s financial institution. I understand that if the terms of this contract are not met, collection activity may occur.
After 12 months your membership will conclude and not renew automatically. If you choose to renew your membership within 14 days of your end date, KPE will waive the enrollment fee.
Short Term and Flexible Memberships:
By joining Kilauea Phys Ed and choosing the ‘temporary’ 1, 3, and 6 month memberships, I thereby agree to the terms and conditions outlined here. I understand that my membership will terminate after its chosen term. I understand that the short term and flexible membership plans cannot be canceled, and no refunds will be given more than 24 hours after purchase.
12-Month Autopay Memberships
If for any reason you need to cancel your membership prior to the end of your current 12-month membership term, a $150 early-termination fee will apply, before canceling your monthly subscription payment obligation, please note that all past due balances incurred during your active membership must be paid before a cancellation can be processed.
All cancellations must be completed IN PERSON at the Kilauea Phys. Ed. location by completing a written/signed cancellation form OR by sending a written/signed cancellation request via certified mail to our corporate address: Good to Go Living Inc. 6186 Helena Lane Kapaa, HI 96746.
Please note that ‘Paid-In-Full’ memberships may not be canceled, and no refunds will be given for unused portions of the 12-month duration.
For all paid-in-full-memberships no refunds will be given after 24 hours from the purchase date No credit will be given for unused gym time.
Any member ship may be refunded within 24 hours of purchase. After that period early cancellation fees and policies apply.
Age Requirement: Individuals who wish to become a member of Kilauea Phys Ed must be at least 15 years old. Minors 15-17 are not permitted to purchase day passes and must purchase a full membership.
Parental Presence: A parent or legal guardian must be present at the time of sign-up for all members who are under 18 years of age.
Independent Use: Members who are under 18 years of age may use Kilauea Phys Ed without a parent or guardian present if they are a member and their parent or guardian has signed the liability waiver and financial guarantee and membership agreement.
This policy is in place to ensure the safety and well-being of all members, particularly minors while using Kilauea Phys Ed facilities.
