By joining Kilauea Phys Ed I agree to the terms and conditions outlined here. After the initial month’s membership fee +$99 enrollment fee are paid, I agree to a monthly autopay charge of $99 monthly via a credit or debit card beginning 30 days after the initial payment. I agree that I am a duly authorized account holder on the credit/debit card issued by the presented card’s financial institution. Memberships will conclude at the end of the 12-Month term. Members may opt to renew their membership within 14 days and the enrollment fee will be waived. Members may also opt to put their memberships on ‘hold’ for a fee of $4.99 per month by requesting a ‘hold form’ at our reception desk.  

Cancellation policy

If a member needs to cancel their 12-month membership before the agreed upon term, a $150 early termination will apply. Cancellations must be made in person at our location by completing and signing a cancellation form. NOTE: Short term memberships, 10 Anytime Passes, 3 month, and 6 Month memberships may not be cancelled or placed on ‘hold’.

Refund Policy

Any member ship may be refunded within 24 hours of purchase. After that period early cancellation fees and policies apply. 

Parental Presence: A parent or legal guardian must be present at the time of sign-up for all members who are under 18 years of age.

Independent Use: Members who are under 18 years of age may use Kilauea Phys Ed without a parent or guardian present if they are a member and their parent or guardian has signed the liability waiver and financial guarantee and membership agreement.
